Abstract   In order to assess the efficacy of alternating scheduling chemotherapy on the outcome of pa- tients with small cell lung cancer(SCLC), we carried out an alternation combination chemo- therapy of CAV with EP. Twenty eight eligible patients with SCLC were treated with an alternating induction chemotherapy consisting of cyclophosphamide(1,200 mg/m), adriamycin (50 mg/m), and vincristine(1.4 mg/m) on day 1 of the 1st, 3rd, and 5th cycle with etoposide (120 mg/m on day 1 to 3) and cisplatin(60 mg/m on day 1) of the 2nd, 4th, and 6th cycle every 3 weeks for six treatment cycles. All responding patients received thoracic irradiation and pro- phylactic brain irradiation after completion of chemotherapy. Six patients(40%) achieved complete response(CR) in 15 limited disease(LD) and three(38%) in 13 extensive disease(ED). Overall CR rate was 32%(9/28). Overall response rate was 78%[93% (14/15) in LD and 61%(8/13) in ED]. With median follow-up period of 10 months (4-50+months), the median survival was 10 months (13 months in LD, 7 months in ED). And median survival in patients with CR, PR, and non-responder were 20, 10, and 6 months, respectively(p<0.05). The median time to progression was 7 months in CR patients. The major toxicities included leukopenia 79%(22/28), nausa & vomiting 75%(21/2S), alopecia 46%(13/28), infection 21%(6/28), and neuropathy 11%(3/Z8). There was no treatment-related fatal death. These results suggest that alternation chemotherapy with CAV and EP in SCLC is as effective as other conventional chemotherapy.
